When coming to our office for your first visit you will need to bring the following:
Driver's License/Photo ID
Insurance Card (Only applies if you have dental insurance)

If at all possible, print out the New Patient Registration and Medical History forms. This way you can fill out all of the information required for your first visit at your convenience. (All forms must be completed entirely before being seen.)
